Can the individual dental plan be purchased as a stand-alone plan?

No. In order for someone to get the individual dental plan, they are required to enroll into an individual medical plan and select the dental at the initial time of enrollment. There is no opportunity to add the dental later or be reinstated if canceled.

How does ODS determine the effective date of an individual policy?

Effective dates are always the first of the month following receipt of all underwriting requirements including premium, complete application with future payment method selection and health history information (all dates of treatment, medications, and physicians).

When will the individual plan ID card(s), policy, and member handbook(s) be received?

If the individual plan ID card(s), policy, and member handbook(s) have not been received within 30 days from underwriting acceptance, please contact the eligibility representative at (503) 265-5606 in the Portland area or (800) 578-1402, extension 5606, toll free between 7:30 a.m. and 4:00 p.m.
